This session demonstrates how Record Locator Service, RLS, is a powerful tool for healthcare providers to access a comprehensive view of a patient's medical history, regardless of where past care occurred. Teresa Reitman's care team used CommonWell RLS to quickly retrieve her medical records during an acute asthma attack while visiting her father 70 miles from home. Using query-based document exchange and FHIR data exchange, her care team coordinated her care, and Teresa accessed her own health data through a consumer application. Thanks to this coordination, Teresa returned to her normal life.

Come see how the 360X IHE approved protocol facilitates efficient closed-loop referrals and care transitions. Referral/transfer requests are initiated and accepted/declined, and the receiving caregivers receive patient information to provide efficient care. Once completed, the receiving providers return clinical outcome documents to close the loop. This technology is in general availability with several EHR vendors.
